An Oregon man tragically died after falling into a blender at a meat distribution plant.
On Friday, Portland’s Hugo Avalos-Chanon was cleaning a meat blender in Clackamas, Ore. at Interstate Meat Distributors. At around 11:45 a.m., Avalos-Chanon fell into the running blender, and though another worker triggered the emergency stop it was too late.
His official cause of death was blunt-force injuries and chopping wounds.
The plant had reportedly been cited numerous times for safety violations, but even after Friday’s incident it remains open.
Avalos-Chanon worked for DCS Sanitation Management, which was also being investigated by the Occupational Safety ad Health Administration (OSHA).
Interstate Meat Distributors is said to be cooperating with the investigation, but have not given additional statements to the media.
