Over the last several years we have seen a growing problem with Big Pharma pushing medicines onto perfectly healthy people, either through scare-tactic ads or mandated by complicit politicians. Indemnified by the Homeland Security act and isolated by the United States Vaccine Court, the pharmaceutical companies have developed a reckless disregard for the safety of their products and little concern if the products being sold are really effective.
As a result, a great deal of harm is being visited on our children in the name of corporate profits, such as measles, mumps, and whopping cough epidemics in populations of children theoretically vaccinated against those illnesses, and the extreme harm being cause to tens of thousands of young girls by the HPV vaccine, in theory to protect 350-400 girls from ever getting this extremely rare disease.
The mandatory 1976 Swine Flu vaccination campaign had to be canceled when the vaccine killed fifty times as many people as the flu itself. Likewise, the United States Vaccine Court confirms that over 100 people have been killed by the measles vaccine last year while CDC confirms nobody has died of the measles itself in the US in the last ten years!
Last night I got a call from a panicked father in California who is being pressured by the school health authorities to have his daughter be given the Gardasil shot. Even though California’s mandatory school vaccine law Senate Bill 277 has yet to be voted on, medical practitioners are offered financial inducements to jab those needles into as many girls (and now boys) as possible.

I attest that I have been made aware of the risk of serious side-effects of the vaccine I am administering to my patient.

See the attached documentation for the risks and damages that have occurred from delivery of said vaccines to patients across the globe.
If the practitioner really truly believes their products are safe and effective, they should not have any reason not to sign this agreement, which is a direct contract that bypasses the indemnification of the Homeland Security Act.
If the practitioner does hesitate, or outright refuses to sign, then you have all the proof you need that the shot being prescribed is not safe and that the practitioner knows it.
